Grade 6 Physical Exam

The School Health Code of the Commonwealth of Pennsylvania requires a physical examination for children in sixth grade. The law gives you the option to either have this exam performed by your private physician at your expense; or have this required exam performed by the school dentist free of charge.

The school district recommends that this examination be done by your family physician since s/he can best evaluate your child's health and assist you in obtaining necessary treatment and corrections.

Please provide a copy of a report from your child's most recent Physical Exam from his/her Physician. Acceptable examinations include (but are not limited to) those performed during the summer vacation, exams administered in conjunction with summer camps, or exams administered during the 5th grade school year. If you need a form, they can be found under "Nurses Forms"

Scoliosis Screening notification

The 28 PA Code, Chapter 23.10(b) states "A scoliosis screening test shall be administered to students in grade six and grade seven and to age-appropriate students in ungraded classes". The purpose of the scoliosis screening is to detect any abnormal curvature of the spine. The 6th-grade scoliosis screening is generally done as part of the required physical examination in the spring of their 6th-grade year. All students who have not submitted documentation of a scoliosis screening completed during their 6th-grade physical exam will be screened. Scoliosis screenings for 7th-grade students will be conducted during physical education classes in the spring of their 7th-grade year. If a student does not pass the scoliosis screening a referral notice will be sent home.

Grade 7 Dental Exams

The School Health Code of the Commonwealth of Pennsylvania requires a dental examination for children in seventh grade. The law gives you the option to either have this exam performed by your private dentist at your expense; or have this required exam performed by the school dentist free of charge.

The school district recommends that this examination be done by your family dentist since s/he can best evaluate your childs health and assist you in obtaining necessary treatment and corrections.

Options for completion of this requirement:

  1. Please provide a copy of a report from your childs most recent dental visit with his/her dentist. Acceptable examinations include (but are not limited to) those performed during summer vacation, exams administered in conjunction with summer camps, or exams administered during the 6th grade school year. if you need a form, they can be found under "MS Nurse Forms".
  2. The SMILES Mobile Dentist Clinic is a mobile dentistry team that visits school and provides dental care to students during the school day. They come once a year and forms will be sent home for you to sign up your child prior to each offered clinic. Often, children can be seen at a reduced cost or sometimes even free!
  3. The School Dentist will do dental screening only in the Spring of each school year if your child has no other means of receiving dental care. Please note that this is only a screening.
