Google Excuse Form

Please complete the Google Excuse Form for the 24/25 School Year.

If you have a doctor's note, please bring it to the Middle School Main Office, or you can fax it to: 610-987-4240

If you know your child will be out of school ahead of time, you can list a future date on the Google excuse form.

*Please note, If you are going on vacation, you will need to complete an Educational Trip Form (please stop by the Main Office)
